General Office Policies

Please arrive 15 minutes prior to your first appointment to allow for the necessary paperwork. Please bring your insurance cards. Co-payments are collected on the day of service. Cash, checks, and major credit cards are accepted. We do not validate parking, but there is a parking structure adjacent to the building. Accepted Insurance Plans We participate with Medicare and many PPO insurance plans. We are not contracted with any HMO insurance plans. We do accept cash for the services that we provide.
